  • Safety Eye Wear Authorization Form (To ensure a current form is always displayed, open the link with Internet Explorer as each form is needed).  Employee brings the form and their current prescription to a Dispensing Location to select their safety eye wear. Any cost not covered by the employer is the employee’s responsibility payable to the eye care provider.  An eye examination may be required to obtain a current eyeglass prescription.  A “current” eyeglass prescription is within 2 years of the issue date and is not expired.  A list of eye care providers for safety glasses is available at https://psc.walman.com/safety/web/walman/map2.htm  Click on a state to view a list of available offices (Offices are periodically added or removed from the list of dispensing locations so check the availability with each new order for safety eye wear).
