- Safety Eye Wear Authorization Form (To ensure a current form is always displayed, open the link with Internet Explorer as each form is needed). Employee brings the form and their current prescription to a Dispensing Location to select their safety eye wear. Any cost not covered by the employer is the employee’s responsibility payable to the eye care provider. An eye examination may be required to obtain a current eyeglass prescription. A “current” eyeglass prescription is within 2 years of the issue date and is not expired. A list of eye care providers for safety glasses is available at https://psc.walman.com/safety/web/walman/map2.htm Click on a state to view a list of available offices (Offices are periodically added or removed from the list of dispensing locations so check the availability with each new order for safety eye wear).
FOLLOWING AN ON-THE-JOB INJURY OR ACCIDENT:
- Complete the electronic-First Report of Injury (e-FROI). Do not complete the PDF (paper copy) of the First Report of Injury unless the e-FROI system is down. An e-FROI submitted online allows us to track and report incidents more easily, and saves supervisors time in completing the form, as some field self-populate.
The supervisor should provide the employee with paper copies of the following documents upon notification of a work-related injury:
- Temporary Prescription Drug ID Card (complete the middle portion of this form)
- Minnesota Workers’ Compensation System Employee Information Sheet
- Designated Medical Providers
